This is a part-time or full-time position working a flexible schedule of 20-40 hours/week.
Our offices are located 20 miles from downtown Denver in Evergreen, Colorado.
This position will travel throughout the foothills and Denver-Metro.
About the position This position will:
Build and manage relationships with existing and potential referral sources, serving as a trusted advisor and partner with case managers, social workers, medical assistants, and other key leaders in hospitals, physician practices, skilled nursing facilities, rehab facilities, and assisted living facilities.
Provide effective marketing and education to achieve strategic goals and leverage market potential.
Keep detailed and timely records of outreach activities and create a schedule of future activities.
Execute strategic tactics designed to meet budgeted admission/census goals for all programs.
Understands referral source/community needs and makes recommendations regarding meeting those needs and offering solutions.
Facilitate communication between intake, our clinical care team, and referral partners to ensure seamless care delivery and timely response to referral partners and patients/caregivers.
Provides customer service to ensure that performance meets expectations and coordinates with the Director of Clinical Services to resolve any service issues.
Assist with obtaining Election of Benefits, as requested, for Hospice, Palliative Care and Home Health.
Meet individual and team admission goals.
Support community outreach opportunities as appropriate.
Performs other duties as required and necessary to ensure the success of the Agency.
Work 20-40 hours a week with flexible hours from 8:
30 am to 5 pm.
Travel to existing and potential referral partners in the foothills and the Denver Metro.
Have the freedom to work remotely with weekly visits to our office in Evergreen.
This is an hour position starting pay in the $32.
00 - $35.
00 per hour range depending on experience and hours.
Be eligible for quarterly bonuses when meeting referral and admission targets.
About you You must (required):
Have a bachelor's degree or equivalent.
Have a minimum of two (2) years' in referral development, sales, or marketing.
Have excellent verbal and written communications skills.
Be able to assess and respond to the needs of patients and families in varied settings.
Be detail-oriented and have strong organizational skills.
Work well as part of a team and have the ability to multi-task and prioritize in a fast paced environment.
Provide documentation of full COVID-19 vaccination or documentation requesting a reasonable exemption for medical or religious reasons.
Have proof of legal right to work in the United States Ideally, you will (preferred):
Have previous referral development or marketing/sales experience in healthcare, home health care, or hospice.
Have knowledge of Home Health, Hospice and Palliative patient care and philosophy.
Have knowledge of assessment protocol & admission criteria.
Have knowledge of insurance reimbursement, Medicare, and Medicaid.
Have a basic knowledge of Microsoft Office programs (Outlook, Word, Excel, PowerPoint).
Have previous experience with CRM software.
